Complete your consent form while you wait so we can get you checked in as soon as we’re available.

BEFORE YOUR PIERCING, PLEASE KNOW:

A valid, government-issued photo ID is required.

• Driver’s license
• State-issued ID
• Passport

CLIENTS UNDER 18

Clients under 18 must be accompanied by a parent or legal guardian, and additional identification is required.

Acceptable identification for minors includes:

• Birth certificate
• Driver’s license
• State-issued ID
• Driver’s permit
• Passport

The parent or legal guardian signing the consent form must have the same last name as the minor OR provide documentation showing their relationship or legal guardianship.

Different last names?
The easiest option is to bring the minor’s birth certificate showing the accompanying parent’s name.

Please make sure you have all required identification with you. We cannot perform the piercing without the appropriate documentation.

WHILE YOU’RE HANGING OUT

• Please make sure you’ve had a snack or a proper meal before your appointment.

• Piercings are VERY anatomy dependent. We’ll evaluate your anatomy and discuss appropriate options with you before your piercing.

• Please try not to touch or handle jewelry on the displays.

• Have questions about jewelry, placement, healing, or downsizing? We’ll be happy to help once we’re available.

CHILDREN & ANIMALS

For everyone’s safety, children and animals may not be left unattended in the studio at any time.

Children accompanying you must be able to remain seated in the designated seating area of the piercing room for the duration of your service. If you feel your child may have difficulty remaining seated during your appointment, please bring someone with you who can supervise them while you are receiving your service.

Thank you for your patience — we’ll be with you shortly :)
